Professional Care Services supports program participants and their families by providing quality case management services throughout north-central West Virginia. Eligible older adults and disabled individuals living independently or with a caregiver can access a wealth of information and services designed to support a successful in-home care experience. Our clients benefit from quality care integration that helps them enjoy living in their preferred home environment for as long as possible.

Our Case Management Services

Community Service Referrals, Care Plans & Advocacy

Case management services may encompass as few or as many supports as required. From offering assistance with maintaining independence to comprehensive access to multiple medical, personal care, and social support services, each case management care plan is tailored to best meet the needs of each individual.

Eligibility

Case Management Service

Professional Care Services provides quality Case Management Services through the West Virginia Aged and Disabled Waiver (ADW) Program and the Traumatic Brain Injury Waiver (TBIW) Program (ages 3 to adult). In-Home Personal Care clients who are physically and/or mentally frail, as well as their caregivers, may be eligible to receive case management services (as appropriate) after being approved to receive services. Interested individuals may apply through the West Virginia Department of Health and Human Services. Please note that case management services are a required condition for Medicaid Waiver Program participants. Case management services are directed by you and will be centered on you, to ensure your unique needs are met.

What Are the Benefits of Case Management Services?

Your Professional Care Services Case Manager can help ensure a successful long-term transition to In-Home Care so you can maintain your independence. Our goal is to coordinate care, help clients and caregivers navigate the application and renewal processes involved with accessing services, and provide information and assistance getting the resources you need to avoid nursing home or long-term care placement for as long as possible. Case Management services are conveniently provided in the comfort of your own home and may include the following:

Application Assistance
Your case manager can provide aid when applying to either waiver program, help guide you through the initiation process, and assist you with the annual medical and financial eligibility reviews.

Needs Assessment
Your case manager will complete an initial Needs Assessment to identify your individual care requirements, determine if your ongoing needs have changed, and ensure services continue to meet your needs by contacting you monthly.

Developing a Service Plan
Your case manager will work with you to identify your needs and preferences and develop a service plan designed to address these needs.

Service Coordination
Your case manager can link you to other services such as ordering medical supplies and equipment, helping you sign up for home delivered meals, and provide assistance accessing other available community resources.

Advocacy
Your case manager is on your side to help ensure your rights are protected and can advocate on your behalf when dealing with issues related to the Personal Attendant (PA) Services agency. Assistance can also be provided to help you with the Fair Hearing Process, if needed.
