Long Term In-Home Care

West Virginia’ s Medicaid Waiver Program enables financially and medically-eligible seniors and individuals with disabilities to receive the care necessary to remain within the home environment. The Aged and Disabled Waiver Program is an alternative to nursing home care. The program provides in-home assistance with activities of daily living and other areas of personal need. Eligibility criteria include:

  • 18 and Older
  • Eligible for Medicaid
  • Physician-Ordered Services Due to Physical and/or Mental Impairment

This program helps individuals stay independent in their homes and communities. The nursing home alternative program works in conjunction with Professional Care Services as your community-based service provider. Coverage includes general health services, as well as coverage for services deemed necessary for your loved one to remain in the home.

Medicare and the WV Medicaid
HCBS Waiver Program for Home Care

Medicare covers in-home care only if certain criteria are met, including being homebound and requiring skilled care. Those who qualify for Medicare home health care services may require additional services. If eligible for Medicare services, this coverage can supplement the type and amount of services available. If ineligible for Medicare home care, your loved one may still be eligible to receive quality in-home care through West Virginia’s Medicaid HCBS Waiver Program. Covered services vary based on several factors, but may include:

  • Personal Care – Hygiene, Grooming & Toileting
  • Homemaker Services – Meal Preparation, Grocery Shopping, Light Housekeeping & Laundry
  • Daily Living Assistance

All personal care services are supervised by a registered nurse.

An assessment of needs and a plan of care will be developed based on your loved one’s individual circumstances. All care plans are reviewed by our highly trained and experienced registered nurses every 6-12 months (or sooner) to help ensure that the correct and most appropriate services are being provided to best assist our clients. If your loved one has reached a point in life where daily living tasks have become a burden, yet he or she still has a strong urge to remain active in the home and community, the Medicaid Waiver program can help provide the necessary financial assistance to help make a worthwhile dream a reality for income-eligible and medically-eligible individuals.
